Norwood Crossing is a nonprofit healthcare organization specializing in quality senior care. Our facility is located on the northwest side of Chicago in the quaint neighborhood of Norwood Park. Our organization is seeking a RN Medicare Coordinator to join our  team. ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • The RN Medicare Coordinator will be responsible for the coordination of all aspects of patient care from admission, under the supervision of the Director of Nursing and Administrator.   
  • Oversees the 3rd floor rehab unit, and communicates level of care for new resident to all disciplines. 
  • Ensures that resident's present/potential problems are identified and prioritized; realistic goals are established and nursing intervention is appropriate. 
  • Assesses the achievement or lack of achievement of desired outcomes.
  • Responsible for all level of care changes within the facility.
  • Works closely with all departments, and notifies & notifies appropriate staff members when a level of care change has been made. 
  • Other duties as assigned.

 

QUALIFICATIONS:

  • Must be a Registered Nurse with current Illinois License.
  • Able to read, write, speak and understand English.
  • Must have basic computer skills
  • Must be an analytical thinker whom can make accurate clinical judgments at a moment's notice.
  • Ability to make independent judgments, follow directives and receive constructive criticism.
  • Ability to deal tactfully with superiors, subordinates, other facility personnel, residents, family members, visitors, government agencies/personnel and the general public.
  • Must be in good physical and emotional health.
  • Ability to stand and walk for prolonged periods of time.
  • Ability to pass the pre-employment, drug testing, and back evaluation processes.
